Are You Eligible?

Benefit-eligible employees enrolled in the medical plan who elect to participate in the GLP-1 Voluntary Program.
Participants who meet clinical criteria as determined by an intake assessment questionnaire and/or meeting with the Strata GLP-1 medical personnel.

How to Enroll

1. Verify Your Eligibility — Confirm your identity with your name, date of birth, and member ID. We verify your enrollment in seconds.

2. Complete the Intake Assessment — Answer the clinical screening questionnaire online. Takes about 5 minutes. No paper forms.

3. Meet Your Care Team — A nurse practitioner reviews your intake and works with you to create your personalized treatment plan.

Once elected, this benefit remains in effect for 12 months after initial registration and may only be changed if you experience a qualifying life event.
